Eligibility Requirements:
You must be between the ages of 13 and 17.
A parent or guardian must complete and submit a permission form.
Submission Options:
The completed form can be turned in to staff at any of our branch locations.
Alternatively, it can be uploaded using the form below.
Once submitted, our Programming Department staff will review the form and send you an invitation to join CPPL Teens.
